There is a particular relief in a solution that asks nothing of you. Take the shot, and the appetite that ran a person’s life for decades goes quiet. For millions, that is not a gimmick. It is the first thing that has ever worked.

This month Medicare began helping pay for some of these GLP-1 drugs, and in the same stretch a study circulated reporting that people were more likely to be hired after losing weight on them. Semaglutide, tirzepatide, and their kin have moved from the diabetes ward to the center of American life.

The mechanism is real and powerful. The drugs mimic a gut hormone, slow the stomach, and quiet the appetite signals in the brain. The weight comes off because the hunger does. But two facts sit beside the success. A meaningful share of the loss is lean muscle, not only fat. And when people stop, the weight tends to come back — the extension of the landmark STEP-1 trial found participants regained about two-thirds of their lost weight within a year of stopping. These are chronic drugs, not cures.

The strongest case for them is that lifestyle advice failed millions for reasons that were never willpower. A metabolic and hormonal environment tilted against a person is not corrected by a pamphlet, and a drug that finally moves the needle is medicine doing its job, with real cardiometabolic benefit.

The strongest case against is that a nation reaching for an injectable to fix a problem built by its food supply is treating the smoke and ignoring the fire. A finding that people get hired after losing weight is not a health outcome at all. It is a measure of how much a body’s shape governs a life, and coercion does not become therapy because it comes in a pen.

Who gets the drug, who keeps affording it, and who loses coverage and regains the weight all track the wallet. The benefit is as unevenly distributed as the condition it treats.

The drugs work. The question is what they are working around. The metabolic environment that made them necessary — the food, the movement, the sleep, the exposures — is the one a resilience-first approach through Detox America keeps insisting can still be changed. A shot that quiets hunger does not quiet that question.
